The report found that the driver error was reported as a critical factor in over 94% of the crashes.

The recognition error, which includes driver’s inattention, internal and external distractions, and inadequate surveillance, was the most (41%) frequently assigned critical reason within the driver-related critical errors. Decision error such as driving too fast for conditions, too fast for the curve, false assumption of other’s actions, illegal maneuver and misjudgment of gap or others’ speed accounted for about 33% of the driver-related critical errors.
